Common questions about Availments.

You may request for an LOA for your medical availment thru any of the following channels:


  • Via our Member Mobile App. Our app is available for downloading via AppStore or Google PlayStore; just search for InLife Benefits; or,
  • Email us at loarequest@inlifebenefits.com.ph


For faster processing of the LOA, please ensure that you have your Doctor's Request or Diagnosis -- particularly for tests and procedures -- ready and attached to the app or email request.


We highly encourage members to request an LOA before going to a medical facility. This is to avoid waiting time in the hospital or clinic.

Reimbursement is subject for evaluation and the terms of your benefit.

File your PhilHealth Form to maximize your benefits. It is the member’s responsibility to submit the required PhilHealth documents.


If not filed, the equivalent PhilHealth benefit amount will be deducted and shouldered by the member; so be sure to complete your filing to avoid additional costs.
