Accomplished Claim Form
Medical Certificate
Original Sales Invoice/s (SI) with BIR Authorized To Print (ATP) and TIN issued to the Insured/patient, for hospital bill and professional fees
Detailed Statement of Account (SOA) from the Hospital
Accomplished Auto-Credit Arrangement Form

Additional Requirements

If Surgical Case:
Surgical Records or Operative Report
Original Official Receipt (OR) with BIR ATP and BIR Number
For Medicines bought outside of hospital during confinement period:
Certification of medicine/s non-availability from the hospital

Fill out Reimbursement Claim Form on our Mobile App

Fill out, completely and correctly, the Reimbursement Claim Form. Upload the necessary documents on our Mobile App. Download the App

Claims will be assessed by our Claims Department

After submission of your fully accomplished reimbursement claim form and the required documents, our Claims Department will take time to verify and assess your claim.

Check Status of your Claim

A notification will be sent to you via our Mobile App. This is to update you of the status of your claim or if additional requirements will be needed for submission.

Funds Credited to Enrolled Bank Account

If claim is approved, this will be credited to your enrolled Bank Account within 14 working days. Please ensure you have submitted the accomplished Auto-Credit Arrangement (ACA) Form. Day 1 is counted on the day Claims has confirmed completion of the Claims requirements.
