Basic Requirements

DEATH CERTIFICATE: An Original Philippine Statistics Authority (PSA) / National Statistics Office (NSO) copy of the Death Certificate will be required for submission. If date of death is less than six (6) months, Certified True Copy of Local Civil Registry is acceptable. In case the death of the Life Insured occurred outside of the Philippines, the Death Certificate must be authenticated by the Philippine Consular Office in the country/state where death took place.
BIRTH CERTIFICATE OF THE LIFE INSURED: An original PSA/NSO copy should be submitted.
CERTIFICATE OF EMPLOYMENT (COE) OF THE LIFE INSURED
INLIFE BENEFITS APPLICATION FORM: Signed by the Insured, if any

Additional Requirements

Additional Requirements for New Employees:

MEDICAL RECORDS: Including consultation, hospital records
COPY OF DAILY TIME RECORD / RECORD OF ATTENDANCE / PAYSLIPS: From effective date of coverage up to date of death
ATTENDING PHYSICIAN’S STATEMENT/S: This must be accomplished by the Physician/s who attended to the deceased and must be notarized.

Additional Requirements for New Dependents:

Additional Requirements if death is due to an accident:

AUTOPSY REPORT, if performed
POLICE REPORT (Original or Certified True Copy)
MEDICAL RECORDS SUCH AS ADMITTING HISTORY OR MEDICAL ABSTRACT, if insured was brought to the hospital
COPY OF DRIVER'S LICENSE, if insured was driving at the time of accident

Fill out completely the Claims Online Form

Access the Claims Online Form here and upload the necessary documents.

Claims will be assessed by our Claims Department

After submission of your fully accomplished reimbursement claim form and the required documents, our Claims Department will take time to verify and assess your claim.

Check Status of your Claim

An email will be sent to your registered email address within 15 days from the time of filing. This is to notify you of the status or if additional requirements will be needed to be submitted.

Funds Credited to Enrolled Bank Account

If claim is approved, this will be credited to your enrolled Bank Account within 14 working days.

Please ensure you have submitted the accomplished Auto-Credit Arrangement (ACA) Form.

Day 1 is counted on the day Claims has confirmed completion of the Claims requirements.
